BackgroundPalliative sedation and analgesia are employed in patients with refractory and intractable symptoms at the end of life to reduce their suffering by lowering their level of consciousness. The doctrine of double effect, a philosophical principle that justifies doing a “good action” with a potentially “bad effect,” is frequently employed to provide an ethical justification for this practice. Main textWe argue that palliative sedation and analgesia do not fulfill the conditions required to apply the doctrine of double effect, and therefore (...) its use in this domain is inappropriate. Furthermore, we argue that the frequent application of the doctrine of double effect to palliative sedation and analgesia reflects physicians’ discomfort with the complex moral, intentional, and causal aspects of end-of-life care. ConclusionsWe are concerned that this misapplication of the doctrine of double effect can consequently impair physicians’ ethical reasoning and relationships with patients at the end of life. This study examines the relation of language use to a person’s ability to perform categorization tasks and to assess their own abilities in those categorization tasks. A silent rhyming task was used to confirm that a group of people with post-stroke aphasia (PWA) had corresponding covert language production (or “inner speech”) impairments. The performance of the PWA was then compared to that of age- and education-matched healthy controls on three kinds of categorization tasks and on metacognitive self-assessments of their performance (...) on those tasks. The PWA showed no deficits in their ability to categorize objects for any of the three trial types (visual, thematic, and categorial). However, on the categorial trials, their metacognitive assessments of whether they had categorized correctly were less reliable than those of the control group. The categorial trials were distinguished from the others by the fact that the categorization could not be based on some immediately perceptible feature or on the objects’ being found together in a type of scenario or setting. This result offers preliminary evidence for a link between covert language use and a specific form of metacognition. (shrink)

The phenomenon of recitation took its place in Islamic sciences in early period, and it has maintained its vitality in the agenda until today. The primary factor that ensures this is the fact that the different aspects contained in the recitations can be suitable as a source. Because the discipline in question can maintain its vitality by constituting a source for sciences such as tafsir, hadith and fiqh. This vividness of the phenomenon of the recitation in the classical period is (...) also seen in today's academic studies. In addition, the fact that a great number of researches have been carried out about recitation both in our country and abroad constitutes an evidence from a different perspective. In this sense, it is known that many scholars contributed to the science of recitation by producing qualified autonomous studies related to the discipline. One of the figures who contributed to the science of recitation is Abû ‘Alı̄ al-Fāārisı̄ı̄, whose work was in demand in classical madrasah tradition. Considering the geography and the time period in which the author lived, it is seen that the recitations have spread to wide areas of their present places. Fāārisı̄ has presented a study, based on the idea of expansion. In addition, a number of works belonging to the author are mentioned since he is a philologist. Therefore, the author comes to the fore with his works by being shown as a reference for many studies compiled after him. On the other hand, although the author has not remained his study exclusive to the science of recitation, he elaborated different aspects of recitations by intensively processing them. In particular, the requirement to comply with the Arabic language, at least in one aspect, which is one of the three basic criteria that the recitation must have in order to be considered valid, led the author to focus intensively on the language factor in recitations. That the author synthesized the phenomenon of language with the recitations attracts attention. Therefore, some determinations were made based on the recitations on the author’s study based on the importance of referring to the recitations of the Quran at building determinations when making a linguistic analysis. The aim of this study is to reveal the method and the quality of the recitation that Fāārisı̄ discussed in the study titled al-Ḥujja fı̄ ʿileli'l-qirā ʾati's-sebʿa, written by Abû ‘Alı̄ al- Fāārisı̄ (d. 377/987) who lived in the 4th century Hijri. High-dose chemotherapy and autologous stem cell transplantation is used to treat some advanced malignancies. It is a traumatic procedure, with a high complication rate and significant mortality. ASCT patients and their carers draw on many sources of information as they seek to understand the procedure and its consequences. Some seek information from beyond orthodox medicine. Alternative beliefs and practices may conflict with conventional understanding of the theory and practice of ASCT, and ‘contested understandings’ might interfere with patient adherence to the (...) strict and demanding protocols required for successful ASCT.The present study, conducted in Sydney, Australia, examines narrative-style interviews with 10 sequentially recruited ASCT patients and nine of their carers conducted at the time of transplant and three months later. Transcripts were read for instances of mention of alternative advice, and for instances of contested understanding of information relevant to the transplant.Patients and carer pairs expressed closely concordant views about alternative advice. Five pairs were consulting alternative practitioners. Contested understanding was expressed in four domains—understandings of the transplant itself and its underlying theory, of the relationship between the components of the ‘transplant’, of the nature and role of stem cells, and of beliefs about bodily function and life-style. Contested understandings of the transplant treatment were expressed as predominantly personal interpretations of orthodox informationPatients and carers seemed to recognise that alternative and conventional systems were discordant, yet they were able to separate the two, and adhere to each practice without prejudicing their medical treatment. A single case of late, post-transplant repudiation of Western medicine is discussed to emphasise some of the possible determinants of dissonance when it does occur. (shrink)

The genus problem of populism presents one of the most vexing conceptual questions across the social sciences: Some theorists believe that populism is nothing more than an assembly of discursive patterns, while others maintain that populism is a strategy to gain political power. Then there are those that argue that populism is a thin ideology that lacks a coherent set of guiding principles. The paper intervenes in this debate in two ways: First, it offers a methodological apparatus for evaluating and (...) developing contested concepts such as populism. Second, it puts forward and defends the claim that populism can be fruitfully understood as a coherent ideology that rests on four foundational principles. These principles, I will argue, are necessary for explaining the paradigmatic beliefs and dispositions exhibited by exponents of populism. Capacity evaluation has become a widely used assessment device in clinical practice to determine whether patients have the cognitive ability to render their own medical decisions. Such evaluations, which might be better thought of as “capacity challenges,” are generally thought of as benign tools used to facilitate care. This paper proposes that such challenges should be reconceptualized as significant medical interventions with their own set of risks, side effects, and potentially deleterious consequences. As a result, a cost–benefit analysis should be (...) implemented prior to imposing such capacity challenges, and efforts should be made to minimize such challenges in situations where they are unlikely to alter the course of treatment. This, however, need not be possible at once. Ultimately the explicanda are to be replaced by certain counterparts, the so-called explicata, which have to conform to four requirements. They have to be as precise as possible and as simple as possible. In addition, they have to be useful in the sense that they give rise to the formulation of theories and the solution of problems. In this paper, we study the distribution and interpretation of a non-temporal use of the future tense in Italian, called ‘presumptive’ or ‘epistemic’, which we label here PF. We first distinguish PF from its closest modal relatives, namely epistemic necessity/possibility/likelihood modals, as well as weak necessity modals. We then propose an account of PF in declaratives and interrogatives that treats it as a special comparative subjective likelihood modal, and test its empirical predictions. A theoretical lesson drawn from this detailed study (...) of the semantics of PF is that semantics needs sharpened theoretical tools to be able to capture the fine-grained distinctions languages make when it comes to signaling modulated epistemic commitment to a proposition. There are two popular views regarding the origin of critical thinking: (1) The concept of critical thinking began with Socrates and his Socratic method of questioning. (2) The term ‘critical thinking’ was first introduced by John Dewey in 1910 in his book How We Think. This paper argues that both claims are incorrect. Firstly, critical reflection was a distinguishing characteristic of the Presocratic philosophers, setting them apart from earlier traditions. Therefore, they should be recognized as even earlier pioneers of critical (...) thinking. Secondly, John Dewey not only used the term ‘critical thinking’ before 1910, but there were also other authors who used it before him. The meaning of ‘critical thinking’ at the turn of the twentieth century was shaped by various traditions of linguistic usage, including literary criticism, science and medicine, and Kantian philosophy. The Implementation Problem for conceptual engineering is, roughly, the problem conceptual engineers face when attempting to bring about the conceptual change they support. An important aspect of this problem concerns the extent to which attempting to implement concepts can lead to unintended negative consequences. Not only can conceptual engineers fail to implement their proposals, but their interventions can produce outcomes directly counter to their goals. It is therefore important to think carefully about the prospect of attempted implementation leading to unintended (...) negative consequences: what sort of negative consequences can conceptual engineers expect? Are some forms of conceptual engineering more likely than others to lead to such consequences? And is conceptual engineering still viable even given the risk of such consequences? This paper addresses such questions. I begin by outlining different forms of conceptual engineering (Section 1), before examining how they can produce unintended negative consequences (Section 2). I then discuss some implications of the fact that attempted implementation can produce unintended negative consequences, suggesting that, among other things, some forms of conceptual engineering are less viable than others (Section 3). I conclude, though, by considering some of the ways in which conceptual engineering is nonetheless a worthwhile pursuit (Section 4). (shrink)

The ability of the mind to conceptualize what is not present is essential. It allows us to reason counterfactually about what might have happened had events unfolded differently or had another course of action been taken. It allows us to think about what might happen – to perform 'Gedankenexperimente' (thought experiments) – before we act. However, the cognitive and neural mechanisms mediating this ability are poorly understood. We suggest that the frontopolar cortex (FPC) keeps track of and evaluates alternative choices (...) (what we might have done), whereas the anterior lateral prefrontal cortex (alPFC) compares simulations of possible future scenarios (what we might do) and evaluates their reward values. The JTB definition of knowledge has been shown by Gettier to be incomplete: its three conditions are necessary for knowledge, but not sufficient. We argue that the JTB definition can be completed with a very simple fourth condition, namely that the justification for the belief in p must be sufficient to exclude ¬p. It is shown that the resulting JTB+S definition solves the Gettier problem.

The question concerning the meaning of life is important, but it immediately confronts the present authors with insurmountable obstacles from a philosophical standpoint, as it would require us to define not only what we hold to be life, but what we hold to be meaning in addition, requiring us to do both in a properly researched context. We unconditionally surrender to that challenge. Instead, we offer a vernacular, armchair approach to life’s origin and meaning, with some layman’s thoughts on the (...) meaning of origins as viewed from the biologist’s standpoint. One can observe that biologists generally approach the concept of biological meaning in the context of evolution. This is the basis for the broad resonance behind Dobzhansky’s appraisal that “Nothing in biology makes sense except in the light of evolution”. Biologists try to understand living things in the historical context of how they arose, without giving much thought to the definition of what life or living things are, which for a biologist is usually not an interesting question in the practical context of daily dealings with organisms. Do humans generally understand life’s meaning in the context of history? If we consider the problem of life’s origin, the question of what constitutes a living thing becomes somewhat more acute for the biologist, though not more answerable, because it is inescapable that there was a time when there were no organisms on Earth, followed by a time when there were, the latter time having persisted in continuity to the present. This raises the question of where, in that transition, chemicals on Earth became alive, requiring, in turn, a set of premises for how life arose in order to conceptualize the problem in relation to organisms we know today, including ourselves, which brings us to the point of this paper: In the same way that cultural narratives for origins always start with a setting, scientific narratives for origins also always start with a setting, a place on Earth or elsewhere where we can imagine what happened for the sake of structuring both the problem and the narrative for its solution. This raises the question of whether scientific origins settings convey meaning to humans in that they suggest to us from what kind of place and what kinds of chemicals we are descended, that is, to which inanimate things we are most closely related. The epistemic value of a political procedure—such as democracy or a civil trial system—depends on how well it performs in arriving at decisions that are correct by some independent standard. A core assumption in the literature on epistemic democracy is that boosting the epistemic value of such a procedure makes it better overall. Even though this assumption seems innocuous (and hence has not been discussed in much detail), we will argue that it is not beyond the pale of reasonable disagreement. (...) Sethi and Schepers have identified an important issue for the global economy: Providing some mechanism for requiring assurance that environmental, social, and corporate governance information provided by a business is accurate and objective. Where they have gone wrong is in trying to change the mission of the United Nations Global Compact. From its inception, the UNGC has been clear that its mission is not to provide such assurance. This article first outlines the background for the historic announcement of the UNGC (...) by the then Secretary-General of the UN, Kofi Annan. Then a summary of the major criticisms of the initiative is provided with a focus on the Sethi–Schepers article. Finally, I argue that the mission of the UNGC, to gain consensus in the global community on the shared values and moral norms that will guide the global economy, is being accomplished, although it is a work in progress. Determinism which denies freedom of action is a common philosophical view. Is the action of such determinists incompatible with Kant’s claim that a rationally willed being “cannot act otherwise than under the idea of freedom” [G 4, 448]? In my paper, I examine Kant’s argument for this claim at the beginning of the Third Section of the Groundwork and argue that it amounts to the assertion that one cannot act while being aware of being guided by invalid principles. Belief in (...) determinism does not necessarily amount to such awareness. A structurally similar argument based on the contrast between deliberation and prediction is examined and similarly found wanting. (shrink)

This article describes the work of negotiating and reinterpreting "standard" protocols and criteria at the level of local practice, using the example of the procurement of human cadaver organs for transplantation. The tension between efforts to starulardize and globalize biomedical science, on the one hand, and fitting these efforts into everyday practices and understandings of practitioners, on the other, results in new constructions of medical knowledge about bodies and persons.

What are the relations among wisdom, virtue, and expertise? Wisdom can be defined broadly as knowledge about how to live well. At the least, the task of living well requires some conception of what it means for a life to be _good_ as well as the knowledge and skill needed to actualize the good in one’s spheres of life. While this idea is easy to assert, it is difficult to examine empirically. This is because the scientific study of wisdom immediately (...) runs up against the challenge of the fact/value dichotomy. While psychological science seeks to study “what is”, the “wisdom” of any given decision, act or person is something that can only be assessed against some conception of the good. Thus, the study of wisdom calls on us to seek ways to bridge the fact/value dichotomy. In this paper, we pursue this goal. We suggest that the study of wisdom requires the integration of at least two forms of inquiry: psychological-empirical analyses of the development of knowledge and skills, and philosophical-conceptual inquiries into what it means to live a good life. In elaborating this approach, we first differentiate the concepts of _wisdom_, _wise decision-making_, and _wisdom-supporting skills_. Then we describe conceptual and _empirical_ tools for assessing the development of wisdom-supporting skills as well as an _evaluative_ framework for assessing the “wisdom” of any given act of decision-making. To illustrate these ideas, we report the results of a study demonstrating how the capacity for wise decision-making can be cultivated through participation in a program devoted to fostering the development of wisdom-supported skills. KeatingA historically conversant reader interested in the current state of discourse regarding Catholicism and American politics will find a good amount of familiar discord. He will discover, for example, that the life issues continue to bedevil. Can a Catholic vote in good conscience for an abortion-rights candidate over a pro-life competitor if that candidate is more supportive of other (...) policies in line with Catholic Social Teaching? This issue has, of course, been with the American Church for some time now, even if it has taken on renewed urgency given the coincidence of the overturning of Roe v. Wade with the presidency of Joseph Biden, the second Roman Catholic to hold that office and a recent convert to pro-choice stridency. The action of the Supreme Court and the battles within the various states will ensure that abortion remains at the political heart of American Catholicism for years to come. One can say something similar about other issues, such as immigration, gun control, and economic justice. None of this will be found to be particularly surprising. The same cannot be said, however, of the return of a more fundamental question, one thought to have been settled since the mid-1960s. That question is whether Roman Catholicism and American liberal democracy are incompatible to the extent that Catholics ought to seek, if possible, another constitutional arrangement, one more friendly to the Catholic cause.Our reader would be familiar with the standard narrative that, while the existence of a medieval Church in a modern polity that upholds religious freedom had been a matter of concern since the dawn of the Republic, it [End Page 991] became serious only in the first half of the twentieth century as the number of Catholics and their influence grew. Until that time, American bishops, such as Archbishop John Ireland (1815–1918), made a special point of speaking publicly on the benefits of American freedom for the Church, just as they made clear that Catholics could be as patriotic as any other citizen. If there were tensions, they came primarily from anti-Catholic bigotry and governmental overreach. Things changed, however, when the point at issue was no longer primarily a religious one of Catholic believers infecting their Protestant neighbors with erroneous doctrine, but rather the threat so many Catholics posed to the fate of American freedom itself. In a postwar America concerned with the spread of Soviet communism, Paul Blanshard found a ready audience for his scholarly anti-Catholic diatribes, American Freedom and Catholic Power (1949) and Communism, Democracy, and Catholic Power (1951).1 Without much difficultly, he pointed to various statements of Gregory XVI and Pius IX condemning the separation of Church and state, the freedom of religious conscience, and democracy as the best governmental form. To top things off, he had Leo XIII's Testem Benevolentiae, an 1899 letter to the Cardinal Archbishop James Gibbons of Baltimore, warning him and his fellow bishops not to speak of the constitutional arrangement of the United States as a political ideal. While Leo had positive things to say about the American system, his admonition followed the traditional line: while constitutional protections of religious freedom are necessary when Catholics are in the minority, the ideal situation is a state that officially confesses and supports the Catholic religion. Of more recent vintage, Blanshard could quote the response of America's best-known Catholic theologian, Monsignor John A. Ryan, to the question of whether non-Catholics would be permitted to practice their religion in this ideal state:If these are carried on within the family, or in such inconspicuous manner as to be an occasion neither of scandal nor of perversion to the faithful, they may be properly tolerated by the State,... Quite distinct from the performance of false religious worship and preaching to the members of the erring sects, is the propagation of the false doctrine among Catholics. This could become a source of injury, a positive menace, to the religious welfare of true believers. Against such an evil they have a right of protection by the Catholic State... During the early 19th century, a peculiarly systematic way of organizing books emerged in Germany. This systematization, which purported to be a rational organization of subject matter, was an outgrowth of the philosophy of Hegel. This article attempts to outline Hegel's organizational practice. It argues that Hegel's encyclopedia was a reaction against the Enlightenment encyclopedia, and that it attempted to restore the systematic mindset of pre-modern reference books. Yet it did this, not in a straightforward fashion, but by developing a (...) method for organizing material that both stresses the interconnectedness of knowledge and promises to be highly scalable. In its attempt to trace the Hegelian organizational method, this article revisits and calls attention to a forgotten work in Hegel scholarship by Michael John Petry, who was the first writer to seriously stress the importance of classification in Hegel's work. Physical and biological measurements might display range values extending towards infinite. The occurrence of infinity in equations, such as the black hole singularities, is a troublesome issue that causes many theories to break down when assessing extreme events. Different methods, such as re-normalization, have been proposed to avoid detrimental infinity. Here a novel technique is proposed, based on geometrical considerations and the Alexander Horned sphere, that permits to undermine infinity in physical and biophysical equations. In this unconventional approach, a continuous (...) monodimensional line becomes an assembly of countless bidimensional lines that superimpose in quantifiable knots and bifurcations. During the 1890s, animal development became associated with glandular activity, with profound implications for pediatric nosology and treatment. The significance of this endocrinological turn of developmental physiology and pathophysiology in part hinges on an often-overlooked continuity with ubiquitous early modern medical thought concerning semen as a recrementitious (reabsorbed) nutrient or stimulant. Mid-19th-century interests in adult sexual physiology were increasingly nerve-centered and antihumoral. Scattered empirical, particularly veterinarian, interests in gonadal developmental functions failed to moderate these explanatory trends. While Brown-Séquard’s rejuvenation experiments (...) still offered no clear starting point for a developmental endocrinology, in 1892 Gaston Variot and Paul Bezançon more explicitly deduced a testicular developmental endocrinological function from various observations on testicular ectopy and a local form of animal “demi-castration.” Ensuing interest in the thyroid, the thymus and in the testicles led to various working conceptions of their respective and putatively reciprocal developmental properties, including the idea of a thyroid–testis axis. From 1896, the pubertal affliction of chlorosis became the subject of multiple opotherapeutic approaches, providing an experimental basis for theories of ovarian internal secretion. Polyglandular therapy, piloted for divergent developmental conditions, remained routine until the 1930s despite the biological inefficacy of many endocrine products. (shrink)

To stimulate research and innovation (R&I), to contribute to the solution of societal challenges and to align R&I with societal values, the European Commission has launched the governance framework of Responsible Research and Innovation (RRI). RRI figures in many high-level EU policies as a means to promote smart growth, and a growing community of R&I practitioners from both the public and private sectors appears committed to it. Although debates on what RRI precisely entails have not reached closure yet, RRI provides (...) an interesting avenue to explore ways of making R&I more societally germane. While recognizing the usefulness of keeping critical reflection on RRI’s meaning alive, we suggest that to make the step from theorizing to implementation, RRI could benefit from a clearer conceptualization. This chapter presents the iterative trajectory in conceptualizing RRI followed as part of RRI Tools, one of a number of EC-funded research projects and support acts aimed at fleshing out what RRI can and should be, and the conceptualization of RRI that this led to. It suggests that RRI is best captured if in R&I governance attention is paid to the five p’s of Purpose, Products, Processes, Preconditions and People, and that further elaborations on the meaning of RRI should happen in dialogue with attempts at practicing RRI. definitions and explanations frequently come together and permeate almost all fields of knowledge. This does not exclude mathematics, even when these definitions hold clear links and close connections with our physical world. Here we propose a rather different perspective. Making operational physical assumptions, we show how it is possible to rigorously reconstruct some features of both geometry and topology. Broadly speaking, assuming this operational and more concrete philosophy we not only are capable of defining primitive concepts like points, straight lines, (...) planes, angles and spaces, but we also go all the way up to more complex constructions. We show that within our method it is also possible to come up with what we call a compass-based topology as well as a normed and a metric space. We hope this operationalist point-of-view can assist other researchers and students to have a clearer understanding of these quite abstract concepts. (shrink)
